A RAID log is the single most important governance artefact a project manager maintains. RAID stands for Risks, Assumptions, Issues and Dependencies (sometimes Decisions). It is the place where the unknown, the uncertain and the blocked are made visible, to the team, to sponsors and to the steering committee.
Most teams attempt to keep a RAID log in a spreadsheet and abandon it within two sprints. The reason is almost always the same: no agreed scoring, no owner, no review cadence, and no link back to the project plan. This template solves all four. Each item carries an owner, a target resolution date, a severity score, a status, and a free-text mitigation. Risks are scored on likelihood × impact (1–5 each), so anything 15+ is automatically a steering committee item.
