RACI matrix template

Free RACI matrix template to end the "I thought you were doing that" conversation. One accountable name per activity, no exceptions.

RACI stands for Responsible, Accountable, Consulted and Informed. It maps every significant project activity against every role, so accountability is explicit before delivery starts rather than argued about after something slips.

The single rule that makes a RACI work: exactly one A per row. Responsible can be shared, Consulted and Informed can be long lists, but accountability cannot be split. Two As means nobody is accountable. Zero As means the activity will quietly stall. This template validates both conditions as you fill it in.

Common failure patterns to check for once populated: a row with no R (nobody is doing the work), a column with As on every row (one person is a bottleneck), and everyone marked C on everything (decision paralysis by consultation).
